1. Personal Information: Provide your personal details accurately, including your name, contact information, date of birth, gender, and PAN (Permanent Account Number).

2. Scheme and Subscriber Information: Fill in your PRAN (Permanent retirement account Number), date of superannuation, and the name of the PrAO (Pay and Accounts Officer) or DTO (Drawing and Disbursing Officer) of the government office where you served.

3. Annuity Option: Choose your preferred annuity scheme from the options mentioned above.
